Childcare and School Vaccine Requirements 

Make sure your child is up to date with vaccines. To beat the back to school rush, don’t wait until  summer break. A good time to get vaccines is during a yearly well-child checkup. Be sure to get an updated SC Certificate of Immunization for your school nurse. Documentation of  vaccination status is required, by state law, for school attendance. Contact your child’s school nurse  with questions or concerns.

Registration for New and Returning Students 

Online registration should be completed for all new and returning students.

It is important to notify your child’s school nurse of any illness or condition that your child has that may require medical support during the school day.  

Medication 

If your child will need prescription medication during the school day, the Permission for School  Administration of Medication form must be completed and signed by your child’s doctor.  

New medication forms are required at the beginning of each school year.

Authorization forms, completed and signed by the physician, parent and student, are required for those  students who have been granted permission to self-medicate and/or self-monitor, in accordance with  the school district’s policy

Special Dietary Accommodations 

A Diet Order is required for students who need special dietary accommodations. This form must be  completed and signed by a health care provider.
